Request personal data

NCRI provides patient data for living patients through its subject access request process.

To complete the data request process NCRI needs to verify the identity of each living patient whose data is requested, and confirm their consent for the data to be released.

Before NCRI can release patient data in response to a subject access request, it is obliged under legislation to consult an appropriate health professional as health-related data must not be provided if it would be likely to cause serious harm to the physical or mental health of the patient.

  • NCRI requires the name, address and contact number for the appropriate medical consultant or GP, in order to request that they make this assessment.

Form document checklist

The following documents are required:

  • A signed consent form (if you are requesting data on behalf of someone else)
     
  • A state-issued photo ID for each patient, for example a passport, Public Services Card or driving licence.
     
  • A utility bill not more than six months old to confirm each patient’s postal address. This could be a gas, electricity, television or broadband bill. Alternatively, official correspondence from a public body, bank, credit union or insurance company is also accepted.

Information about the files

  • Clear images of original documents should be supplied. 
  • NCRI accepts PDF and JPG/JPEG file types only with size up to 2MB.
  • Please ensure that you have all the required documents before filling out the form.
